I've discovered where the #4 body mount is has rusted and is not connected to the frame anymore. Has anyone repaired/replaced this area of the body?

So for an 81 the body mount goes through the frame into the body. It is not like a small tab has rusted off or broken the frame its self is rusted out at that point.

How big is the damage? Extent.

I would suggest that you seperate the frame from the body and check it. You can have a piece welded back on that is not a big problem if the body is off the frame. You could have more damage between the body and the frame.

Dirt gets into the bushings and grinds down the coating on the frame and exposes the metal to corrosion. Even in Texas you can see that because after the motion of the truck stops the water and dirt are trapped inbetween the surface and it drys very slowly that way.

Clean the frame and coat it again with something. POR 15 or Automotive paint or something. Enamal or POR before you consider any overcoating like a "Under Coating". Frankly, POR-15 or Automotive paint is plenty protection for the next 20 years. Anything more just adds a lot of weight.
